Resulting in the ‘7 Habits of Highly Effective eHealth Enabled Integrated Care implementation teams’ this project is based on two systematic literature reviews and a research mission across Europe visiting 13 regions, interviewing 150 individual health and social care professionals on the impact of implementing e-health in order to facilitate the integration of various health and social services for European elderly living independently at home.
